Am I required to provide any information to AllPatient?

You have control over whether to provide any information to AllPatient. You can opt out of providing information by not using the portion of the Services where the information would be requested.

Most of the features of the Services require you to have an Account with us. If you choose to create an Account, you must also specifically choose to provide a minimum amount of personal information associated with that Account. Should you prefer to not provide any such information, you do not have to create an Account.

What information do we collect from users and how is it used?

  • Registration. Before you can utilize Services, we may ask that you register with the Services and provide us with your email address, a password, your first and last name and other contact information and personal details. We request this information for identification purposes, to communicate with you regarding your account, and to facilitate the functioning of certain aspects of the Services. We may keep this information indefinitely.

To fully utilize the Services, you may be required to fill out forms that will contain personal information such as your name, address, telephone number, social security number, account information, employment, health status, historical or current health information, and other personal information relevant to your health insurance coverage, diagnosis, and treatment.

This information may be shared with your treating physician(s), whose name and/or contact information that you provide to us.

  • URL and IP addresses. Like many other websites, AllPatient collects information about users’ utilization and navigation of our Services. This information helps us to design our Services to better suit our users’ needs.   • If you have requested a consultative service, we may use information about you to fulfill such request (including providing that information to third parties, including your treating physician, primary care physician, and other medical professionals you identify that you have interacted with). We may also at any time review your case, your medical records from prior to and after your interaction with the Services, and any records created as a result of services received. We may request courtesy notices from your treating physician(s) and/or request additional medical records from them, including records pertaining to care you received after receiving the services. We may review these records for, among other purposes, reviewing the quality of service you received, reviewing the quality and nature of service provided by your treating physician, including to better understand the course of treatment for your condition(s) including information regarding outcomes and costs. We will take care to minimize personally identifying information in this process. What security measures does AllPatient employ?

We utilize these unique identifiers to verify the user’s identity and eligibility, in order to protect our members from the release of sensitive or personally identifiable information to unauthorized users. Does AllPatient support Do Not Track?

Many newer web browsers may have a “Do Not Track” preference that transmits a “Do Not Track” header to the websites you visit with information indicating that you do not want your activity to be tracked. AllPatient does not currently take actions to respond to Do Not Track signals because a uniform technological standard has not yet been developed.

What is our policy on children users of our Site?

We do not knowingly collect or maintain personally identifiable information from persons under 13 years old, and no part of our Services is directed to persons under 13. IF YOU ARE UNDER 13 YEARS OF AGE, PLEASE DO NOT USE OR ACCESS OUR SERVICES AT ANY TIME OR IN ANY MANNER. If AllPatient learns that personally identifiable information of persons less than 13 years old has been collected without verifiable parental consent, then AllPatient will take the appropriate steps to delete this information.
